Because we highly value your time, our intake forms are provided below for your convenience. Please print out and fill them out prior to your appointment and bring in all forms for a quick and easy check-in. If you are unable to access these forms then they will be provided upon your arrival, but in this case please arrive 20-25 minutes before appointment time to not take up your valuable therapy time with paperwork.
